Minnesota Allergy Society
Standard Immunotherapy Dosing Form Project
March 28, 2006
The Minnesota Allergy Society, representing allergists across the state, has responded to requests from primary care doctors to standardize allergy shot forms and administration recommendations. Taking into consideration the national work of the Academy and College of Allergy, Asthma and Immunology, a committee was formed in 2005 to address the issue. Committee Chair Richard Morris, MD and ten allergists worked throughout the year to create the forms and recommendations. The Society voted to adopt the plan. Primary care providers will be mailed the information in a packet. When new allergy vaccine is required for a patient, the allergist will also include the new forms. Click on the links below for a printable copy of the forms or contact Cheryl Sasse at 651-223-9565 cheryl.sasse@alamn.org.
Dr. Richard Gebhart, past president of the Minnesota Academy of Family Practice, requested this action several years ago. "Our nurses are going to love using this new system. It will reduce our phone calls to the allergists, reduce waiting time for the patients because of those calls, and improve patient safety. It’s a winner all the way around" said Dr. Gebhart.
