Description: Modeled after the Healthy Learners Asthma Initiative, the Saint Paul Public Schools has developed their own asthma initiative, which was piloted in the 2002-2003 school year in 4 elementary schools. In the 2003-2004 school year, the program expanded to 31 elementary schools and 3 junior high schools. The project expanded to all Saint Paul Public Schools in 2006. The goals of the Saint Paul Asthma Initiative are similar to those in Minneapolis. In its early phase, the Saint Paul project has focused on increasing awareness among school staff of students who have asthma, obtaining current information on severity for each child with asthma, improving assessment in the health office, offering performance-based teaching to children with asthma, increasing contact with parents, and increasing contact with healthcare professionals.

Description: The Goodwill loans used durable medical equipment to anyone in need for a period up to six months free of charge. Goodwill/Easter Seals repairs, cleans and sanitizes the equipment before each loan. They also accept donations of used durable medical equipment.

Eligibility: The program is currently offered through the MPLS Public Schools and affiliated clinics.
Description: In partnership with the Healthy Learners Board, a commmunity-based asthma initative has been expanded to include 86 primary and middle schools during the 2001-2002 school year. An evaluation of a pilot project in 7 schools has been completed. Noteworthy achievements of the pilot include "increased capacity within partnering community clinics, abridging services to connect families and students to health care options, increased capacity with the District's health offices, and a ten-fold increase in the use of asthma action plans (AAP). The purpose of the pilot was to develop a blueprint for a population based asthma care and control system with public schools in Minneapolis and St. Paul being a key component of the overarching strategy".

Description: Working with the Legal Aid Society of Minneapolis (LASM) and the Southern Minnesota Regional Legal Services (SMRLS) the MMLA has developed a video (copyrighted in 2000) for housing consumers. The theme of both is how to get a landlord to make repairs and do upkeep on commercial units to improve indoor aid quality. Materials are printed at a 6th grade reading level, although intended audiences include health care providers, parents, school teachers, and school health nurses. All major and minor ethnic and linguistic groups are target, notwithstanding the fact that the video is produce in English only, with same or similar information available in fact sheet form in Spanish, Hmong, Russian, and Somali. Description: The Minnesota Department of Health has collaborated with the American Lung Association of MN in establishing the Minnesota Asthma Coalition (MAC). The creation of the MAC serves as a unique opportunity for individuals and groups to come together and work on asthma in a collaborative approach by promoting awareness, prevention and culturally sensitve education, ensuring access to state-of the-art-care, and providing monitoring, analysis, and dissemination of information. A network of seven regional asthma coalitions has been set-up across Minnesota, to provide suport for local asthma awareness and prevention efforts throughout the state.

Description: Using an action plan takes the guesswork out of treating asthma episodes. The action plan outlines how you can control asthma without always going to the doctor's office or the hospital. Using an action plan created just for you may also help you maintain your activities without suffering an increase in asthma symptoms. Following an asthma action plan can help prevent and reduce inflammation in the bronchial tubes. This can also help prevent permanent bronchial scarring called "lung re-modeling". An asthma action plan helps to: 1) Take the guesswork out of treating episodes by telling you what to do when you or your child has an asthma episode. Often, you can treat symptoms without going to the doctor's office or the hospital, 2) Prevent or reduce the severity of episodes by letting you know what to watch for and how to treat the episode early, 3) Control asthma symptoms so that you or your child continues to be active with fewer asthma problems. Share this information with your health care provider, including a sample of the asthma action plan, and ask your provider to make an asthma action plan for you or your child. Your provider can access on on-line version of the asthma action plan on the Minnesota Department of Health website at http://www.health.state.mn.us/divs/hpcd/cdee/asthma/ . Once an asthma action plan has been created for you or your child, you can follow the plan to manage you or your child's asthma.

Description: The Minnesota Department of Health Asthma Program has established an asthma surveillance system to better understand and describe the burden of asthma in Minnesota. Eligibility: For asthma, a referral from a school system, social worker, medical facility or physician nurse practitioner is required. The client must have an identified health care provider from whom they have previously received medical services.
Description: Home-based clinical services for persons with asthma across the life span are primary. The focus is on disease management versus crisis management. Whole family services are provided to address issues beyond those that come up in the traditional medical delivery system. Target population: Focus is on residents in Hennepin County (less than 1% of the client base resides in Ramsey County).

Description: In October 2001, the American Lung Association of Minnesota in collaboration with the Healthy Learners Board received a two-year planning grant from the Centers for Disease Control and Prevention to develop a strategic plan related to childhood asthma in Minneapolis and Saint Paul. A coalition representing various community sectors including environmental health, childcare, schools, health care, health plans, public health, social services, and community organizations was formed to develop this strategic plan. The Centers for Disease Control and Prevention then granted the American Lung Association of Minnesota five years of funding to implement the plan (September 2003 through June 2008). The Project has been successful in leveraging additional resources for this plan from the federal and state agencies, health plans, pharmaceutical companies, and foundations.
